At any age, it is critical to have a plan for the kind of medical care and support you want today, tomorrow, and during a time of medical crisis. Having support in place as you navigate the health care system is extremely important and a crisis could leave you too ill to make your own health care decisions.
Planning for your health care is an important step toward making sure you get the medical care you would want (and noting what you don’t want), if you are unable to speak for yourself and your designated representatives are carrying out your plans on your behalf.

The Getting Started package includes one-to-one phone or online video appointments with our board certified patient advocate to guide you in:

Identifying and designating a Health Care Proxy in your support network
Understanding what important documents are critical to have in place

Professional Fees: This flat-fee project involves 2 hours of professional work and the full package payment is due at the time of purchase. Payments by check and credit card are accepted.
Patient advocacy services are not included in any Advance Care Planning Guidance Package and will be charged separately. These patient advocacy services will be included in a separate service agreement provided by Beacon Patient Solutions LLC and signed by both the client and Beacon Patient Solutions LLC staff. Beacon Patient Solutions LLC does not provide financial or legal advisory services.

The Putting It Together package includes one-to-one phone or online video appointments with our board certified patient advocate to guide you in:
Assessing what you want and do not want for your Care:
Every Day
In An Emergency
Identifying and designating a Health Care Proxy in your support network
Understanding what important documents are critical to have in place

Compiling Your Advance Care Plan (ALL legal documents must be prepared by an attorney and are not a part of services provided by Beacon Patient Solutions LLC)
Preparing to have conversations about your care interests with important members of your support network
A suggested timeline for revisiting / updating your plans
Storage options
